Personal Injury and Public Liability Insurance

There may be up to two insurance options available to club members or those particpating in club organised events.

Refer to bottom of this page for relevant policies and certificates of currency (CoC).

Important notes:

  • Medicare gap is not covered by injury insurance or private health cover (by law).  Furthermore, some insruance covers do not cover for any treatment that has a medicare item number.

  • Health care costs and claims should be undertaken in the following order:

    • 1) Personal private health insurance

    • 2) Hockey Australia Personal Injury Policy (can also be used to cover above what exceeds or not covered by Private health insurance) - note that claims should be notified to insurer within 120 days of injury.

    • 3) UNSW Group Personal injury insurance policy - note that claims should be notified to insurer within 30 days of injury.

Option 1: Hockey Australia Insurance

If you are a registered member of Hockey NSW and you have suffered an injury whilst participating in an official hockey activity (including training, games or other sanctioned activities) please follow the simple steps below to lodge a Personal Injury Claim.

How to lodge a Personal Injury Claim

 

Personal accident claims

All claims must be notified to Sportscover within 120 days of the accident, injury or damage occurring. 

  • Step 1: Log onto Sportscover Australia and complete the Claim Form Request. This will enable Sportscover to record your intention to claim and allow their office to issue you with all the appropriate claim documents you require to submit your claim.
  • Step 2: Complete as much of the claim form as possible and send to secretary.unswhockey@gmail.com so the "Official Report" page can be completed by a club official.  Either you or the club can arrange for the witness report page to be filled out (e.g. by coach, captain, team member, umpire etc).  The form will then be returned to you to email to Sportscover (please cc the club secretary).
  • Step 3: Once you have completed all sections of the claim form and have returned the completed claim form to Sportscover you will then receive your Claim Number and Internet Access PIN via Post or Email.
  • Step 4: Once you have received your PIN from Sportscover you can monitor your claim progress by visiting the Sportscover website and hit the claims link or you can contact Sportscover direct on the Claims Hotline phone number – 1300 134 956

Public liability and management liability

Claims made against you by members of the general public for personal injury or damage to their property that arise from the conduct of your business, are regarded by insurance companies as extremely serious.

If this occurs, it is essential that under no circumstances should any admission of liability or offer to pay, be given to the person concerned. To ensure that your interests are fully protected, it is important that you notify Marsh within 48 hours of the occurrence at sport@marsh.com.

Option 2: UNSW Group Personal Accident Insurance

This is available to all club members undertaking a hockey club sporting activity (competition or training).  This includes summer competition hockey players not registered with Hockey Australia.

UNSW Sports Insurance policy requires the club to submit / initiate a report.  Usually our club secretary can do this for you.  

To Claim:

  1. Club Executive/Team Manager (TM)/Coach completes Arc Sport's Incident Report.
  2. Claimant (injured person) fills out Personal Injury Claim Form, except for page 8.
  3. Claimant sends filled form to Club Executive/TM/Coach (person that filled out the Incident Report).
  4. Club Executive/TM/Coach signs page 8 and sends back to claimant.
  5. Claimant sends form and supporting evidence** to A&HClaims.AU@chubb.com
