Purpose: To ensure La Trobe University affiliated clubs are following legal (Consumer Affairs) and affiliate (La Trobe Sport) requirements.

Timeline: Form released to clubs on 29/08/2025, clubs to submit form and documents by 01/11/2025*

• Due to the differing nature of club financial years, AGM’s, etc. clubs may submit the form after this date, pending La Trobe Sport approval. You must notify Hayden in writing if this will occur. 
• If AGM date after November timeline, this form must be completed within 14 days of AGM

PROCESS

1. Please visit the submission form prior to starting to submit
2. View all information you need, and collate this
3. Once all information is collated, please upload through the form
   1. It is very important that all information is collated prior to submitting, there have been instances where forms have been kept open for lengthy times – which has caused the form to show an error when submitting.
