Maccabi Junior Carnival

 

I am signing up to participate in the 2025 Maccabi Junior Carnival in Perth

I understand and agree that:

  • I can only participate in Carnival if I comply with Maccabi's Code of Conduct (which I have read) and other Integrity Policies (which I can read HERE), and any other rules and directions made by Carnival managers. Those other rules might include additional rules about:
    • travel and social media use;
    • behaviours that embody Maccabi's Values, including:
      • demonstrating integrity and fair play in all interactions on and off the field;
      • treating other athletes, opponents, officials, and spectators with respect at all times; and
      • being inclusive;  
  • I am responsible to read those policies and rules and become familiar with my obligations under those policies;
  • if I have any questions about individual policies or rules, I need to ask Maccabi for more information;
  • if I want to make a complaint or notify Maccabi about a dispute with someone else about conduct relating to Carnival that is not in accordance with Maccabi’s Integrity Policies, I must follow Maccabi’s Grievance Resolution Guidelines;
  • if I don't comply with Maccabi’s Integrity Policies, Maccabi may discipline me. This might include, but is not limited to:
    • requiring me to apologise to someone
    • withdrawing an award that I have won
    • suspending or ending my participation in Carnival
    • suspending or ending my membership of or participation in Maccabi or a Maccabi Club; 
  • if a complaint or dispute involving me is referred to a Maccabi Tribunal, the decision of that Tribunal will be final and binding on me; and  
  • if a complaint involving me involves a serious allegation (such as alleged criminal conduct or child abuse), Maccabi may need to report the complaint to external agencies, including the police and the relevant State or Federal Government Department, and in that case Maccabi does not need to obtain my consent to that report.

I have honestly and fully disclosed to Maccabi anything about me that is related to any Integrity Policy that would be relevant to me participating in Carnival, and in particular I confirm that:

  1. There are no criminal charges against me before any court;
  2. I have never been convicted for or found guilty of sexual offences, offences related to children or acts of violence;
  3. No disciplinary proceedings have ever been brought against me by an employer, sporting organisation or similar body involving child abuse, sexual misconduct or harassment, other forms of harassment or acts of violence;
  4. To my knowledge there is nothing else related to me that Maccabi needs to be informed about that might be relevant to any risk to Maccabi members, employees, volunteers or participants, or Maccabi's reputation; and
  5. I will notify my Maccabi State President immediately if any of the statements I have made in clauses (1) to (5) above are no longer true.

If Maccabi asks me to, I will complete Integrity training within the required timeframe.

All details I have provided in this form are true and correct at the time of registration.

I have read and agree with the refund and cancellation policies as set out in the Registration form.

For parents of athletes under 18 or who do not have legal capacity

I have read the registration form with my child and have explained the meaning and importance of everything said in the form to them.

To the best of my knowledge, everything that my child has stated or confirmed in the form is true.

I acknowledge and agree that, if Maccabi disciplines my child for a breach of any rules and requires them to return home during Carnival, I will be responsible for paying, or reimbursing Maccabi, for any expenses involved in my child returning home.

I have read and agree with the refund and cancellation policies as set out in the Registration form.
