Membership

By registering for or participating in this dragon boat event:

• you will become a member of Dragon Boat Queensland (DBQ) and by extension the Australian Dragon Boat Federation for the period of the event.
• you agree to be bound by the policies of DBQ including the National Integrity Framework and Fair Play Code that cover such matters as Child Safety, Member Protection, Compeition Maniputlation, anti-doping and the standards of behaviour expected by everyone involved in sport in Queensland.

Fitness & Medical Treatment Consent

I certify that I am medically and physically fit and able to participate in dragon boating and associated activities and have not been advised otherwise by a qualified medical person.

I hereby consent to receiving medical treatment which may be deemed advisable in the event of injury, accident and or illness during this event.

I acknowledge that this declaration will be used and relied upon by the Club, Dragon Boat Queensland Inc., event holders, sponsors and organisers of events in which I may participate as evidence of my fitness and ability to participate and I agree to be bound by it.

Risks of this Sport

I acknowledge that:

• dragon boating can be an inherently dangerous sport; and
• the risks of serious injury, death or accident are not only inherent to paddlers, but are also present for volunteers and spectators.

I have voluntarily read this warning and understood this warning and accept and assume all the risks inherent or otherwise of participating in dragon boating, associated activities of the Club and/or volunteering in this event.

I acknowledge that this Waiver and Release of Liability form will be used and relied upon by the Club, Dragon Boat Queensland Inc., event holders, sponsors and organisers of events in which I may participate and I agree to be bound by it.

In consideration of permitting me to participate in this event, I hereby take the following action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

• Waive, Release and Discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me or my travelling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Dragon Boat Queensland Inc. (DBQ), the Club, the DBQ &/or Club’s directors, officers, employees, volunteers, representatives and agents, the event holders, event sponsors, event directors and event volunteers;
• Indemnify and Hold Harmless the entities or persons mentioned in paragraph (A) from any and all liabilities or claims made by other individuals or entities as a result of any of my actions during this event.

This Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

Other permissions

I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers and or assigns.
