Engadine Gymnastics Club Inc

I hereby give permission for my Self / Son / Daughter / Ward, to participate in gymnastics classes with Engadine Gymnastics Club Inc.

I request that you accept this application. In consideration of your acceptance of the above application for tuition and family membership of your Club, I agree to be legally bound for myself, my heirs, executors and administrators to waive and release the committee and members of the Engadine Gymnastics Club Inc, their agents, representatives and employees from any and all claims or rights claimed for damages for injuries or loss suffered by me or other members of my family, whether of a personal nature or otherwise directly or indirectly in receiving tuition, competing in events, training and traveling to and from any such periods of tuition, competing in events, training or volunteering as a helper.

I further authorize Engadine Gymnastics Club Inc. Instructors or Committee members In the event of an emergency or non-emergency situation requiring medical treatment, grant permission for any and all medical and/or illness, until such time as I can be contacted.  This permission includes, but is not limited to, the administration of first aid, the use of an ambulance, and the administration of anesthesia and /or surgery, under the recommendation of qualified medical personnel. In this even I agree to pay all such expenses incurred on behalf of my son / daughter / ward or other family member.

I acknowledge that there are inherent risks of injury in the participation of gymnastics.  Also I have read, understand and agree to abide by the rules of the Engadine Gymnastics Club Inc.

Equipment Duty Roster Policy & Agreement

I hereby agree to fulfill my duty in setting up and packing away the gymnastic equipment as indicated on the roster.

I understand that Engadine Gymnastics Club Inc. is a not for profit organization and requires all its members to participate in this activity as part of the club rules.

My choices for the roster are:-

Tuesday nights – Set-Up - commences at 7.00p.m   (usually takes approx. 1 hour)      

Thursday nights – Pack-Up - commences at 8.00 p.m. (usually takes approx. 1 hour )  

or  No Preference - (meaning either day is suitable)   

Depending on the number of families enrolled, the average commitment is (the choice of evening for) two (2) consecutive weeks per term. 

Kindly note that we will endeavour to roster you on the evening of your choice, wherever possible.
