Team List Declaration Form 2022 Southern Highlands Hockey
 

Clubs must complete a Team List Declaration form for each of the teams they are nominating in the 2022 Southern Highlands Hockey Competition.

Teams that have not declared their Team List, including graded players, using this form by 6pm Monday 21 February 2022 will not be included in the competition.

At the end of this form is a link to return to the beginning and submit a new team list.
