Queensland Athletics

Membership Application & Declaration

By submitting this application, I apply for membership of Queensland Athletics through the Club I have selected and declare that I am eligible to participate in athletics in accordance with the applicable rules and eligibility requirements of Queensland Athletics and Australian Athletics.

I agree to be bound by the Constitution, Rules, Regulations, Policies and By-Laws of Queensland Athletics, Australian Athletics and World Athletics, as amended from time to time, including applicable codes of conduct, integrity, disciplinary and anti-doping requirements.

I acknowledge that participation in athletics and related activities involves inherent risks and voluntarily accept those risks. I agree to the waivers, releases, exclusions of liability and indemnities contained in the Australian Athletics Membership Application and Declaration – Terms and Conditions.

I acknowledge that, where applicable, I may be subject to drug testing conducted under the Australian anti-doping framework and administered by Sport Integrity Australia.

I acknowledge that my name and competition results may be published as part of the normal administration, reporting and promotion of the sport.

I consent to photographs and video captured by Queensland Athletics, my affiliated Club, or their authorised photographers or media personnel being used for legitimate sporting, promotional and communication purposes, subject to applicable Queensland Athletics policies.

Athletics North Qld

I hereby make application for membership of Athletics North Queensland Incorporated with the Club listed on this registration form and declare that I am eligible to participate in activities according to the eligibility laws set down by the By Laws of Athletics Australia.  I agree to abide by all the Rules and By Laws of Athletics North Queensland, Memorandums, Articles and By Laws of Athletics Australia and Constitution and Rules of the World Athletics as amended from time to time.  (Copies of this available on Athletics North Queensland website).  I consent to undergo drug testing under the auspices of ASADA. I consent to my name and/or image being used in publications, displayed on the website/social media sites of ANQ Affiliated Clubs & Athletics North Queensland, Queensland Athletics, Athletics Australia and World Athletics.

Sarina & District Athletics Club Inc.

I hereby make application for membership of Sarina & District Athletics Club and declare that I am eligible to participate in activities according to the eligibility laws set down by the By Laws of Sarina & District Athletics Club.  I agree to abide by all the Rules and By Laws of Sarina & District Athletics Club, and their Constitution.  (Copies of this is available on Sarina & District Athletics Club Website).  All membership registrations are subject to admission or rejection as per the club's constitution.
