Parent/Guardian Supervision
I understand that this is a Come & Try session and that a parent or responsible guardian must remain on site for the entire session. I understand that coaches and volunteers are responsible for delivering and supervising the activity but do not replace my responsibility as the child’s parent/guardian.

Medical Information
I confirm that I will disclose any medical conditions, allergies, injuries, medications, additional needs or other relevant information that may affect my child’s safe participation in the session. I agree to advise organisers of any changes before participation.

Swimming Ability & Water Safety
I understand that Flippaball and water polo are aquatic activities. I confirm that my child has sufficient water confidence/swimming ability to participate, or I have discussed any concerns regarding their swimming ability with the organisers before they enter the water.

Participation & Sporting Risk
I understand that participation in Flippaball and Junior Water Polo involves normal risks associated with swimming, aquatic ball activities and team sports, including accidental contact with other participants. 

Following Instructions
I understand that my child is expected to follow reasonable instructions from coaches, officials and venue staff, including pool and water-safety requirements. Organisers may remove a participant from an activity where necessary for their own safety or the safety of others.
