I give permission for my child to participate in the Albury Comets Softball Club T-Ball Summer Fun Program.

I acknowledge that participation in sporting activities involves inherent risks, including but not limited to falls, collisions, and injuries that may occur despite reasonable precautions being taken.

I understand that Albury Comets Softball Club, its volunteers, coaches, officials, and representatives will take reasonable care to provide a safe environment but cannot eliminate all risks associated with participation.

I confirm that my child is physically able to participate in the activities and that I have disclosed any relevant medical information.

In the event of illness or injury, I authorise program staff to obtain medical treatment for my child if I cannot be contacted immediately.

I confirm that I have read the above Event Waiver in its entirety.*
