Event Waiver
In consideration of my child/ward participating in activities organised by Yvonne Table Tennis Academy (the "Academy") during the Holiday Camp on 29 June to 3 July 2026 and/or 6 July to 10 July 2026, I/we acknowledge and agree to the following:
1. Acknowledgement of Risk
Table tennis and associated physical activities involve an inherent risk of injury. I acknowledge that my child may sustain injury while participating in camp activities, and I accept these risks on behalf of my child.
2. Release of Liability
I/we agree to release, hold harmless and indemnify Yvonne Table Tennis Academy, its coaches, staff, volunteers and agents from any and all claims, damages, losses or expenses arising from my child's participation in the Holiday Camp, except where caused by the gross negligence or wilful misconduct of the Academy.
3. Medical Authorisation
In the event of an illness or injury, and if I cannot be contacted in a timely manner, I authorise the Academy to seek emergency medical treatment for my child. I agree to bear any associated medical costs.
4. Photography and Media Consent
I consent to photographs and/or videos being taken of my child during the camp for use on the Academy's social media, website and promotional materials.
If you do not give consent, please contact the Academy in advance.
5. Code of Conduct
I agree that my child will behave respectfully toward coaches, staff and other participants. The Academy reserves the right to remove a participant who does not comply with the code of conduct, without refund.
6. Collection and Drop-off
I acknowledge that I am responsible for the timely collection of my child at the end of each session. If my child is to be collected by someone other than the parent/guardian listed on this form, I will notify Academy staff in advance.
7. Supervision
Supervision is provided during all scheduled training sessions and during Supervised Rest Time (11:30 am–1:00 pm) for full-day participants. The Academy does not provide supervision outside of these hours.
8. Cancellation Policy
Fees paid are non-refundable unless the session is cancelled by the Academy. In the event of a cancellation by the Academy, a full refund or credit will be offered.
9. Governing Law
This waiver is governed by the laws of Queensland, Australia.
