Use this form to nominate relay teams representing UQ Athletics Club at the upcoming QLD Relays competition on the 14-15 November. This form should be submitted by coaches or relay team managers only. Individual athletes or parents wishing to enter your child/children please reach out to Taige (taige.hooper@outlook.com) to be placed.

Coaches should submit one form per relay team and may submit multiple responses to nominate additional teams. All nominated athletes must hold an active membership and be members of UQ Athletics Club.
