This form is to be completed by any player wishing to seek an exemption to attend a trial at which they would have been considered for Rep Team selection.

Applications for exemption must set out full reasons as to why the player is not available for the relevant selection event. 

Exemption Requests can be submitted up until 2 days prior to respective trials commencement. 

Toowoomba Hockey will review all submissions prior to providing approved requests for exemption to the relevant selection panel for consideration.

If seeking exemption on medical/injury grounds, you must provide a medical certificate to Toowoomba Hockey prior to the commencement of the scheduled trials.
