I am authorised to submit this application for affiliation form on behalf of the group and hereby apply for affiliation with Qrun as an Associate Member of Queensland Athletic Association Limited. In submitting this application, I agree to abide by the Articles of the Association, By-Laws and Rules of the Association.

I acknowledge that it is a requirement of a Qrun group affiliation for all regular participants in the group’s designated training, programs or activities to be registered as individual members of Qrun.
