U16 Nomination Form

Parkes Hockey Association is calling for nominations for our Junior Representative Teams for the following age groups U18s, U16s & U14s Boys and Girls. 

PHI will be entering teams in U/18, U/16, U/14 age groups at HNSW INDOOR State Championships. 

Representative hockey offers all players the next level of coaching and competition as well as the opportunity to make new friends. 

If you are interested in playing INDOOR Junior Representative Hockey in 2026 please complete and submit this nomination form for your age group. If you wish to nominate for more than one age group, you must nominate in each age group. These nomination forms must be completed by the close date below to be considered for selection.
