Thank you for nominating for a position in an NDWHA Masters Representative Team. By nominating and by signing this form, you agree to the following conditions:
1. I understand the dates of the Event (Tournament / Carnival / State Championships) and confirm that I will be available for all scheduled day(s) of the Event.
2. I understand that while there are minimal costs to trial for selection in a Representative team, my ongoing participation in a Representative Team will involve some form of monetary payment to cover team out of pocket expenses such as training field hire, team accommodation (if booked by NDWHA), team nomination fees, turf fees, umpire fees, food, activities, etc. The approximate amount of the payment will be provided at when finalised. Note that due to late changes to the team, player levies may be subject to variation prior to the Event. In such cases, the Team Manager will keep the team informed.
3. If I am successfully selected in a Representative team and subsequently decide to withdraw from the team for any reason prior to the commencement of the Event (other than for a Medical reason - for which a medical certificate will be required), then I agree to pay the set player levy in full without a refund as outlined below
Withdrawals after 31 December 2026 – Nomination fee is forfeited unless an exemption is granted by the Masters Committee and appropriate documentation is provided.
Withdrawals after 13 January 2027 – No Refunds. Total Rep Fee levy is to be paid. Any unpaid rep levy’s will result in players being declared unfinancial and ineligible to play in ANY 2026 competition.
4. The Team Coach has the final decision on the starting line-up for all games and all player interchanges. I understand that depending on the game strategy employed by the Coach, players may not receive equal field time during any or all of the games played at an Event.
5. Any equipment provided to me during trials, training or the Event games (eg playing shirts, bibs, balls, cones, etc.) are to be returned to the Team Coach / Manager as soon as possible.
6. I agree to abide by all requirements and requests set down by the Team Coach / Manager or an NDWHA official at all times, and to the Player Code of Conduct as outlined by Hockey NSW. I understand that any poor or inappropriate behaviour will be dealt with by the Team Coach / Manager. Any serious breaches of conduct will be referred to the NDWHA Executive for investigation which may involve meeting with the NDWHA Judiciary Panel.
7. In the case of a medical emergency, I provide authority for the Coach / Manager or other NDWHA Official to provide any assistance deemed necessary – including a request for an Ambulance
Grading
Welcome to registrations for the 2026/2027 NDWHA Indoor Masters Representative team/s.
If you have not previously represented NDWHA and NDWHA is not your primary association in 2026 you may be required to provide a release form from your primary association. If you have any questions in this regard please provide details when you register.
HNSW registrations expire 31/12/2026. You will need to renew your HNSW registration prior to the tournament.
Dates: Friday 29/01/2027- Sunday 31/01/2027
Nominations close 6 January 2027 - $100 Nomination Fee payable on Registration.
Costs will be determined once full costs are known. You can expect to pay a cost of between $190 - $240. The total cost will be reduced by the nomination fee paid.
Accommodation has not been booked and is the responsibility of players. You will need to source your or own at your expense.

We will also require team Managers, coaches and umpires. Teams will be limited to a maximum of 10 Players.

If you nominate to play in Division 2, 3, or 4, your nomination will be accepted, but placement in your nominated division is not guaranteed.
The following is a guide for division placements:
* Division 1: Premier League, 2nd, higher 3rd
* Division 2: 2nd, 3rd, higher 4th
* Division 3: 3rd, higher 4th
* Division 4: 4th to 7th grade
Team placements will depend on the number of nominations received and your NDWHA ranking. Please consider this when submitting your nomination. If a division receives too many nominations, trials and a selection process will be held by a selection panel.
