As a player representing Maryborough Indoor Hockey, the following conditions will apply. You must:

be Covid-Safe at all times - refer to Government guidelines
make yourself available for all selection trials/training sessions
advise the selectors/coach/manager of any concerns with illness and injury
be honest in your attitude to and preparation for training/games. Work equally hard for yourself and for your team
co-operate with coaches and staff during the selection trials and training.
abide by the Code of Conduct
complete Medical History form and return to team manager upon selection
