Send an email to the Treasurer  (treasurer@maroochyhockey.com.au) with the pdf of the voucher attached. The voucher will then be applied against the seasons fees.

FAIR PLAY VOUCHER

Applying for a Fair Play Voucher 

Parents, carers or guardians can apply for a voucher valued up to $200 for their child, which can be used towards sport and active recreation membership, registration or participation fees.  There is a limit of 1 voucher per child per calendar year which can be used at registration.

NB: If you are successful, and wish to use for your child's hockey Registration with Maroochydore Hockey Club, then please email the Treasurer with a pdf of the voucher so we can give you a discount code to be used during the season's registration process. Vouchers can not be redeemed for cash after fees have been paid but can be held to be used for the following season during registration.

2024/5 Key dates

1 July 2024           — Round 11 Opens

30 May 2025        — Round 11 Closes

6 June 2025         — Round 11 vouchers must be presented to an activity provider.

What do you need to do...

1. Check if your child is eligible

You can apply if your child:

·       is a Queensland resident aged from 5 to 17 years (inclusive)

·       did not receive a FairPlay voucher this year

·       either:

o   holds, or whose parent, carer or guardian holds, a valid Department of Human Services Health Care Card or Pensioner Concession Card with the child’s name on it, or

o   holds a Medicare card.

2. Make sure your activity is available

Search the activity provider directory to check that:

·       your chosen activity is listed

·       there is a registered provider near you.

·       If the fees are more than $200, you will need to pay the difference.

3. Read the terms and conditions

Read and understand the FairPlay voucher application terms and conditions.

4. Access the Enquire online system

You will need to register for an Enquire account before you can apply for a voucher.

5. If you are successful

If your application is successful, your voucher will appear on the screen. You can access a copy of your voucher from the homepage and a pdf copy will be sent to your registered email address (you may need to check your junk mail).

 6. Present the voucher

Send an email to the Treasurer  (treasurer@maroochyhockey.com.au) with the pdf of the voucher attached. The voucher will then be applied against the seasons fees.
