Little Cubs – Event Waiver & Parent/Guardian Consent

By registering my child to participate in the Hockey New England Little Cubs program, I acknowledge and agree that:

I understand that hockey and associated activities involve physical activity and carry an inherent risk of injury. I consent to my child participating in the Little Cubs program and acknowledge that reasonable care will be taken by Hockey New England, its coaches, volunteers and officials to provide a safe and appropriate environment.

I confirm that my child is physically able to participate and that I will advise Hockey New England of any relevant medical conditions, allergies or additional requirements that organisers should be aware of.

In the event of an accident, injury or illness, I authorise Hockey New England representatives to provide or arrange appropriate first aid or medical assistance where reasonably necessary. I understand that I am responsible for any medical or ambulance costs incurred.

I acknowledge that a parent or responsible guardian must remain at the venue for the duration of the Little Cubs session and remains responsible for their child outside of the organised activities.

By completing this registration, I confirm that I am the child's parent or legal guardian, or have authority to provide this consent, and agree to the above conditions.
