Registrations for this event are closed.
It is a requirement of all players selected to Hockey ACT and Canberra Regional teams to complete the player agreement and medical particulars form, before commencing competition. Failure to do, may result in removal from their respective team(s).
This form only needs to be completed once per year, and is applicable across multiple teams & programs. Please contact Hockey ACT directly (Lil Mutton) if any details require updating.
