Players must be turning 34 or older in 2021 to participate at the Queensland Masters State Championships in Warwick.

 

Note:  Players eligible to play 55+, 60+ or 65+ may opt to ONLY play trials on Thursday 10 June if they wish.  In this case, DO NOT COMPLETE THIS FORM.

 

Players must attend selection trials at the hockey centre on Sunday 11 April, 7:30am - 9:00am and Sunday 18 April 7:30am - 9:00am.

Teams compete on Friday, Saturday and Sunday 11 - 13 June.  We expect the last match to be complete by 3.30pm.

 

Trip cost indication is $370-$390 plus uniform and will include 3 nights accommodation at Granite Belt Brewery, Stanthorpe.  Minimum requirement is to stay Thursday, Friday and Saturday nights. 
Players can make arrangements to pay in instalments provided that all payments are received by Friday 28 May  at the latest.

The $50 deposit payable to Gold Coast Hockey Women's Masters upon submission of this form is NON-REFUNDABLE.
