By registering for this activity, I acknowledge that dragon boating involves physical exertion and inherent risks, including but not limited to slipping, falling, capsizing, and water-related accidents. I must not be under the influence of drugs or alcohol whilst participating in the corporate Dragon boating event.
Assumption of Risk: I voluntarily assume all risks associated with my participation. I certify that I am physically fit and able to swim 50m or adequately support myself in water (a PDF must be worn)
Safety Compliance: I agree to follow all safety instructions given by the coaches/steerspersons.
Release of Liability: I release the event organizer FSDBC, coaches, and venue from any liability for personal injury, illness, or loss of property.
For Participants Under 18: If the participant is under 18, a parent or legal guardian must review and submit this form. By checking the agreement box, the parent/guardian grants permission for the minor to participate and accepts full responsibility and liability on their behalf.
