New Individual Membership Application Form 2026 - Disabled Wintersport Australia

Thank you for applying for Disabled Wintersport Australia Individual Membership.

In order to help us determine your eligibility we ask that you answer the following questions. Once completed, you will receive a request to provide supporting medical documentation from one of your healthcare providers.

Please allow up to one week from us receiving the documentation for approval of membership. If approved, you will be sent a link to register as a new individual member including payment of the membership fee of $300.
