on behalf of my child, myself and other parent/guardians, that we have read and agree to the comply with the CSJBA Ethics and Expectations Policy including Code of Conduct for Tournament and Representative Team Participation that is located on the CSJBA website; and
my child has/will be registered for a Club to participate in the CSJBA 2026/27 summer junior baseball competition; and
I acknowledge that if not selected in the Premier Series, my child may be selected in the CSJBA Worboys Cup team; and
to my child potentially being photographed and appearing on CSJBA social media and/or website.
