Event Waiver

Carrum Downs Royals Volleyball Club Release & Liability Waiver In consideration of myself and/or my child being allowed to participate in any way with Carrum Downs Royals Volleyball Club on/off season training, including all related events and activities, the undersigned acknowledges, appreciates, and agrees that:

Assumption of Risk:
Participation in this program involves certain inherent risks, including but not limited to the potential for serious injury. While safety rules, equipment, and personal discipline may reduce these risks, the risk of serious injury still exists. Please ensure to follow safety rules instructed by Coaches and other professionalisms within the club.

Voluntary Participation & Responsibility:
I knowingly and freely assume all such risks, whether known or unknown, even if arising from the negligence of the "Releasees" or others. I take full responsibility for my (or my child’s) participation. I agree to comply with all stated and customary terms, rules, and conditions of participation. If I observe any unusual or significant hazard during my presence or participation, I will report it to the nearest official immediately.

Release of Liability:
I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Carrum Downs Royals Volleyball Club, its officers, officials, agents, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the event.

Medical Clearance Policy:
If a player leaves a practice or game for any medical reason, they may not return without written medical clearance from a physician. Additionally, coaches reserve the right to require medical clearance before allowing any player to participate, at their sole discretion.

Photo & Media Release:
I hereby consent, without further consideration or compensation, to the use (full or partial) of photographs, audio, or video recordings taken of my child or family members during Carrum Downs Royals Volleyball Club events. These may be used for documentation, promotional materials, or web content. I release Carrum Downs Royals Volleyball Club and its volunteers from any liability that may arise from such use.
