Disclaimer and Waiver

'The Player' means the person doing this registration. All other players listed will need to download a copy of this waiver and sign separately if they didn't register for hockey in 2026 (through a club and Hockey NSW).

Acknowledgment of and assumption of risk: 

The Player, by agreeing to this form, acknowledges and agrees that -

a) Hockey is a sport in which there is a risk that physical injury may occur; 

b) the Player accepts all risks arising from the Summer Hockey Competition (the Competition) organized by Hills Hockey Club Inc. (HHC); 

c) the Player releases and holds blameless HHC, their servants, agents and voluntary workers, from all actions, proceedings, suits, claims and other liabilities founded in negligence, breach of duty (statutory or otherwise) or howsoever arising for injuries sustained by the Player in the course of the Competition. 

If the Player is under the age of 18 years at the time of agreeing to this form, the parent or guardian of the Player 

a) acknowledges and agrees that Hockey is a sport in which there is a risk that physical injury may occur; 

b) agrees that he or she, and the Player, accept all risks arising from the Competition; 

c) hereby releases and holds blameless HHC,  their servants, agents and voluntary workers, from all actions, proceedings, suits, claims and other liabilities founded in negligence, breach of duty (statutory or otherwise) or howsoever arising for injuries sustained by the Player in the course of the Competition.
