On attendance, all parents must sign a Player Indemnity Form, which includes contact details and the following Indemnity Release and Photo/Video Consent. Please see Lee or a Club member if you do not wish for photos or videos of your child to be used:

Indemnity Release Statement:

I, the above named parent/guardian, with my child/children participating in and/or organising any Benalla Hockey Association (Benalla Hockey Club) or Goulburn Valley Hockey Association (GVHA) hockey matches, training, competitions and associated events (Activities) for the sport named above, which are organised by Benalla Hockey Club (BHC), the GVHA and/or any of their affiliated clubs, hereby acknowledge, agree and confirm the following:

· There are inherent risks associated with the Activities which may result in personal injury (even of a serious nature) to participants. I fully accept and agree to bear these risks.

· To the full extent permitted by law I absolve, release, discharge and indemnify BHC, its committee, employees, representatives and agents (Indemnities) from any and all liability for any personal injury, mental anguish, loss or damage of any kind suffered by me or my child/children, however caused, arising out of my/their participation in the Activities, including without limitation, where caused by any acts of negligence by the Indemnities.

·I authorise Benalla Hockey Club to arrange medical or hospital treatment (including, without limitation, ambulance transportation) if I am not present or in a fit state to do so, and I indemnify BHC Indemnities for all costs associated therewith.

I declare that I have read and understood the above and accept full responsibility for my child’s/children’s safety each time they participate in the above nominated sport, and I waiver any claim I might have on BHC and its affiliated clubs as set out above.

I further agree to abide by all instructions and requirements issued by BHC and/or its affiliated clubs in relation to the Activities.

I understand that I am bound by this indemnity and release from the date below until BHC receives in writing from me, notification that my child/children, are no longer participating in the above named sport and/or club.

Photo/Video Consent:

I, the above named parent/guardian, hereby authorise BHC, GVHA and their affiliates to utilise images, including photographs and video, of my child/children and their names for the purposes of publicity, including in media releases, social media, local newspapers, and promotional flyers.
