COVID 19 WAIVER
I have not travelled overseas / been exposed to a person with COVID-19 in the preceding 14 days. I have not been in a designated COVID hot spot in the last 14 days, or have been in contact with someone from a COVID hotspot in the past 14 days. I agree that I do not feel unwell, and don’t have flu/cold like symptoms such as fever, cough, sore throat, shortness of breath, loss of smell or taste, muscle and joint pain, diarrhoea, vomiting or loss of appetite. I consent information from this form can be used by venue owners and government agencies for COVID-19 contact tracing as part of the public health response. I understand that after signing this waiver, if I present with any Covid-19 symptoms as listed above, I will contact Athletics North Queensland and I will not be able to attend the sessions.
In submitting my entry I accept and understand that I must abide by the rules and regulations of Athletics North Queensland. I acknowledge that I have read, understood and agreed to the information I have provided. I have read and agree to the rules and regulations for the duration of this clinic and will follow any instructions given to me in relation to social distancing and health and safety. I confirm that all of the information provided by me in completing my registration is correct and acknowledge that my registration may be refused if I provide false information.
I consent to my name and/or image being used in publications, displayed on the website/social media sites of Affilated Clubs and Athletics North Queensland, Queensland Athletics, Athletics Australia and World Athletics.
