Academy One Volleyball – Junior Prep Program (Invitation Only) Participation Waiver

Term Dates:

Sunday Training: May 4th – July 6th, 2025
Saturday Games: May 10th – July 5th, 2025
Participant Age Requirement: 10 years or older as at 31st December 2025

1. Assumption of Risk & Liability Release

I acknowledge that participation in volleyball training and match play carries inherent risks, including but not limited to falls, collisions, sprains, fractures, and other injuries. I understand that Academy One Volleyball (A1VC), its staff, coaches, volunteers, and the venue (Shore School) cannot be held liable for any injury, loss, or damage sustained during participation in the Junior Prep Program. Participation is voluntary, and I assume full responsibility for any associated risks.

2. Medical Consent

In the event of an emergency where a parent or guardian cannot be contacted, I authorise A1VC staff to secure appropriate medical treatment for the participant. I accept responsibility for any resulting medical expenses.

3. Code of Conduct

Participants are expected to follow all instructions, club policies, and safety protocols at all times. Failure to do so may result in dismissal from the program without refund. Respectful behaviour towards coaches, teammates, opponents, and staff is required.

4. Media Consent

By participating in the program, I grant Academy One Volleyball permission to capture photographs or videos of the participant during training and matches. These may be used for promotional, educational, or marketing purposes including social media, website, and printed materials. No personal information will be disclosed.

If I wish to opt out of media use, I will notify the club in writing before the start of the program.

5. Fees & Non-Refundable Policy

I understand that the training fee is $50 per session (plus transaction fee), and the game fee is $199 per term (plus transaction fee). All training fees are non-refundable under any circumstances, including illness, injury, or schedule conflict. Game fees may only be credited under specific conditions outlined in the Academy One Volleyball Attendance Policy.

6. Membership Requirement

All participants must hold a valid A1VC U18 Recreational Membership prior to participation in any training or match activity. This is separate from session fees.

By proceeding with registration, I confirm that I have read and understood the terms of this waiver and voluntarily agree to its contents.
